Pemphigus seborrhoicus
86-y-o man with
multiple "seborrheic" lesions on the trunc.
Histologic clue: "the naked epidermis", i.e. the superficial cell layers of the
stratum corneum are gone (due to superficial acantholysis) and the epidermis appears
"nude". Confirmation of the diagnosis by direct immunofluorescence (cf. figure).
Differential diagnosis: superficial impetigo contagiosa, which may look very similar
histologically but contains an abundance of bacteria, immuno-stainable with
anti-mycobacterium bovis BCG, which cross-reacts with many other organisms incl. bacteria.
